Nurse Practitioner - 20 hours/week

Part time 20 hours per week

JOB SUMMARY

The Nurse Practitioner works in collaboration with physicians and interdisciplinary teams as an advanced practice nurse to deliver palliative care and other health services to patients. The Nurse Practitioner will work in the community making visits at patient homes, nursing homes, and assisted living facilities, our inpatient Care Center, and will participate in conducting palliative care consults.

EDUCATION AND QUALIFICATIONS                 

  • Master’s Degree
  • Board Certified Advanced Practice Nurse in the state of Colorado
  • Registered Nurse in the state of Colorado
  • Controlled substance license with current DEA license number
  • Eligible or currently possess a Medicare/Medicaid UPIN number
  • Eligible or currently credentialed at local hospitals
  • Eligible to bill insurance for palliative care and symptom management visits
  • CPR Certification
  • One or more years of working experiences as an NP. One or more years working experience as an RN preferred
  • Certification in hospice and palliative care preferred but not required
  • Knowledge of Medicare, Medicaid and hospice standards of practice
  • Excellent interpersonal communication skills
  • Must successfully pass agency required background checks and drug test

Clinical Manager (RN) M-F 8a-5p

JOB SUMMARY

The Clinical Manager is responsible for supervising clinical care teams that provide patient centered, cost effective clinical services to patients and families residing in their designated primary residence.  Leads and coordinates the provision of services in a manner that promotes a team approach, assures caring and respect for patients/families, meets Medicare/Medicaid conditions of participation and accreditation requirements for hospice and reflects the mission and philosophy of TRU Hospice of Northern Colorado.  The Clinical Manager also contributes to the ongoing professional development and competency of all clinical staff across the department and provides site based leadership influencing culture and atmosphere that is consistent with mission and philosophy of TRU Hospice of Northern Colorado.

 
EDUCATION AND QUALIFICATIONS

  • Must possess a current license/degree in nursing or related health care field
  • A minimum of 2 years supervisory experience in a health care environment, hospice experience preferred
  • Demonstrated competency in management and leadership
  • Excellent interpersonal and verbal/written communication skills

Central Staffing Registered Nurse night shift 8p-8:30am 1 part time position 24 hours per week and 1 full time position 36 hours per week

The Registered Nurse (RN) is responsible for providing nursing services to assigned patients. RNs provide direct patient care, collaborate with the other Interdisciplinary Team (IDT) members within the care team, and with other health care providers in various care settings. Patient care provided by RNs is done in a manner that promotes an interdisciplinary team approach, respects patients/families goals for care, meets Medicare/Medicaid conditions of participation and accreditation requirements and reflects the mission and philosophy of TRU Hospice of Northern Colorado. Upon request, participates in all areas of clinical services.

EDUCATION AND QUALIFICATIONS

  • Must possess a current RN license in the state of Colorado
  • Must have a minimum of one year RN experience – hospice or home health experience preferred
  • Must successfully pass agency required background checks and drug screening Must possess a valid driver’s license, provide proof of insurance (a copy of your “Declarations Page” indicating you have $100k/$300k Bodily Injury/$100K Property Damage coverage) and have reliable transportation
  • Requires travel to multiple work locations

Admissions RN 36 hours/week

JOB SUMMARY 
The Admissions Nurse is responsible for admitting patients/families to TRU Hospice of Northern Colorado in a timely manner upon patient/family and referral source request.  The Admissions Nurse conducts the intake and completes the admission process in a manner that promotes an interdisciplinary team approach to care, respects patients/families, meets Medicare/Medicaid conditions of participation and accreditation requirements and reflects the mission and philosophy of TRU Hospice of Northern Colorado.

The Admissions Nurse is also responsible for sales and marketing of the hospice concept and the benefits of program and services to physicians, discharge planners, case managers and other internal and external healthcare professionals.

EDUCATION & QUALIFICATIONS

  • Must possess a current Colorado RN license
  • Two years’ experience in hospice and/or home health to include knowledge of Medicare, Medicaid, accreditation and hospice and home health standards of practice, preferred
  • Experience in medical marketing/sales preferably in hospice/home care environment
  • Effective interpersonal verbal and written communication skills
  • Must successfully pass agency required background checks and drug screening
  • Must possess a valid driver’s license, provide proof of insurance (a copy of your “Declarations Page” indicating you have $100k/$300k Bodily Injury/$100k Property Damage coverage) and have reliable transportation
  • Requires travel to multiple work locations

CNA - PRN status (days/hours vary depending on availability and need)

Provide patient care in a manner that promotes an interdisciplinary team approach, respects patients/families goals for care, meets Medicare/Medicaid conditions of participation and accreditation requirements and reflects the mission and philosophy of TRU Hospice of Northern Colorado. Upon request, participates in all areas of clinical services.

Education and Qualifications

  • Must possess a current Colorado C.N.A. license
  • Must have a minimum 6 months experience - hospice or home health experience preferred
  • Must be a high school graduate or possess a GED
  • Must possess CPR certification or be able to obtain within 60 days of hire
  • Must possess a valid driver’s license, provide proof of insurance (a copy of your “Declarations Page” indicating you have $100k/$300k Bodily Injury/$100k Property Damage coverage) and have reliable transportation
  • Requires travel to multiple work locations

Unit Coordinator/CNA total 28 hrs/wk (Unit Coord shifts: Sat/Sun 8a-4:30p; CNA: 1 12-hr shift/wk)

Job Summary

This position serves as the Unit Coordinator as well as working one 12-hour CNA shift per week. The Unit Coordinator assists all members of the unit staff and oversees the administrative support duties of the team. Supports patients, families, friends, and members of the community in a manner that promotes an interdisciplinary team approach, respects patients/families goals for care, meets Medicare/Medicaid conditions of participation and accreditation requirements and reflects the mission and philosophy of TRU Hospice of Northern Colorado.
